Disabled Children’s Team (TPA)

Ceredigion Social Services Department is committed to making a positive difference to the quality of life of the people of Ceredigion by safeguarding and promoting the wellbeing, independence and life opportunities of service users.

Within the Children & Families remit, there are several Teams operating within the County in line with a wide range of legislative responsibilities and provisions.

Tîm Plant Anabl/the Disabled Children’s Team is a multi-agency Team of Health, Education and Social Services professionals.  They work with children who suffer from a moderate to severe learning, physical or sensory disability or children who have a chronic illness or complex health needs.  They will assess, plan, co-ordinate and provide services for the child and their family.  Their aim is to minimise the impact of the child’s disability or illness and maximise the child’s integration into society.  Children with a mild disability will be assessed by the Children and Family Assessment and Support Team.

All referrals are to be made through the Department’s Single Point of Access in the first instance.
